Self-Assessment: Awareness, Values, and Mindfulness (AVM Scale)

Purpose

To help individuals reflect on and evaluate their:

Self-awareness – understanding of thoughts, emotions, and behaviors.

Values – clarity about personal priorities and guiding principles.

Mindfulness – presence, attention, and non-judgmental awareness of internal and external experiences.

Instructions:
For each statement, rate yourself honestly on a scale from 1 to 5:

1 = Strongly Disagree
2 = Disagree
3 = Neutral
4 = Agree
5 = Strongly Agree

Take your time and reflect on your experiences, thoughts, and behaviors.

Section 1: Self-Awareness

(Recognizing your thoughts, emotions, and behavior patterns)

  1. I am aware of the emotions I am feeling at any given moment.

  2. I notice patterns in my thinking that influence my behavior.

  3. I understand how my actions affect others.

  4. I reflect on my decisions and behaviors regularly.

  5. I can recognize when I am stressed, anxious, or upset before it escalates.

  6. I am aware of my strengths and weaknesses.

  7. I can identify my habitual responses to difficult situations.

  8. I can separate my emotions from my actions when making decisions.

Section 2: Values

(Clarity about your personal principles and priorities)

  1. I am clear about what is most important to me in life.

  2. My daily actions reflect my core values.

  3. I can articulate my personal beliefs and guiding principles.

  4. I feel internal conflict when my actions do not align with my values.

  5. I make decisions based on long-term goals rather than immediate convenience.

  6. I can prioritize tasks and decisions according to my values.

  7. I feel motivated by a sense of purpose that aligns with my beliefs.

  8. I am able to explain why my values matter to me.

Section 3: Mindfulness

(Present-moment awareness, focus, and non-judgmental observation)

  1. I can observe my thoughts without immediately reacting to them.

  2. I remain focused on the present moment, even under stress.

  3. I notice small details in my surroundings that others might overlook.

  4. I can acknowledge my emotions without judging myself.

  5. I take time to pause and breathe before responding in challenging situations.

  6. I am aware of physical sensations in my body as they happen.

  7. I can bring my attention back to the present when my mind wanders.

  8. I am mindful of how my words and actions impact others.

Scoring

  1. Calculate domain scores: Average the responses in each section.

    • Example: Add up all 8 responses in Self-Awareness and divide by 8.

  2. Interpret domain scores:

    • 4.0 – 5.0 = High awareness/values clarity/mindfulness

    • 3.0 – 3.9 = Moderate, some room for growth

    • <3.0 = Low, potential area to develop

  3. Overall AVM score: Average all 24 items for a general measure of awareness, values clarity, and mindfulness.

Reflection Questions

  • What patterns did I notice in my self-awareness responses?

  • Which values are most important to me right now, and do I live by them daily?

  • How often do I practice mindfulness in my daily activities, and where can I improve?
